Fuel Assistance Program
Documentation required for your Home Energy Assistance Program Application 2025-2026

Please bring copies of the following items for your appointment:
Only those that pertain to you the primary and anyone 18 yrs. or older living with you.
1.____ Applicant Identification
-Driver’s License or Picture ID of head of household.
-Social Security Card for everyone living in the household.
-Birth Certificate for anyone 18 years of younger.
2.___ Proof of gross income for the last 30 days (copies) for anyone 18 years or
older living in the home. This includes:
-Last four paychecks or everyone over 18 years of age in household.
-Social Security Award Letter (Not just direct deposit.)
-Child Support Proof
-Alimony Proof
-SSI
-Emergency Aid to Elderly, disabled and children,
-Temporary Assistance to Families with Dependent Children
-Veteran Benefits
-Retirement/Pension Income Letter (Need stub from check, direct deposit cannot be used for this.)
-Bank Statements for direct deposit income other than social security and pension.
-1099’s for bank interest over $100.
-Interest & Dividend income.
-IRA disbursements.
3.___ Current fuel bills (from previous months of application). They would like 3. If you would like to be considered for the High Energy Benefit Program you must submit you last 12 consecutive bills.
4.___ Housing Costs:
-Electric bill within the last month.
-Telephone/Cable bill within the last month.
-Gas bill within the last month.
Homeowners: -Mortgage bill. -Homeowner’s insurance bill. -Real Estate Tax bill with a total yearly amount on it. -Sewer bill. –Water bill.
Condo owners: -Condo fee bill.
Mobile home owners: -Monthly park rent.
Renters: -Copy of rental lease or letter from landlord that includes their telephone and address. -If Tenant at Will, must submit a rent receipt.
5.___ If you are behind on bills (i.e. mortgage, rent utilities) please submit them also
6.___ If you are self-employed:
Schedule C 1040 Tax Document
7.___ If there is a second home that is used as rental property, you must submit
a 1040 with schedule E. If you own a second home that is used as a vacation
home, you are automatically not eligible for fuel assistance.
NOTE: Anyone 18 years or older living in the home, needs to sign the fuel assistance application. If they are unable to come to the appointment, you will be given an addendum to have them sign. This will be sent to the fuel assistance agency. The application will not be processed by the fuel assistance agency until they receive the addendum.
NOTE: If another name appears on your bills (i.e. a child, guardian, etc.) you must submit a copy of their license or a bill that has their name and address on it.
Please note: The Maximum Annual Gross Income for a one-person household is $51,777and the limit for a two-person household is $67,709. There is local fuel assistance available for those with higher incomes.
The fuel assistance program is administered by Self Help, Inc. Annual recertification applications are mailed directly to participants from Self Help, Inc. The fuel assistance season runs from November 1st to April 30th each year
