U.S. Passport Applications Can be Accepted at the Senior Center U.S. Passport applications are now being accepted at the Westwood Senior Center, 60 Nahatan Street. Carol McCarron, Program Development Director for the Council on Aging, will be available to accept passport applications by appointment only at various hours throughout the week. Applicants may call the Senior Center at 781-329-8799 to schedule an appointment. Applicants must bring a completed, unsigned application form to the appointment. Application forms are available at the Senior Center or by clicking here for the DS11 form. These forms are also available at other local processing facilities (e.g., many local post offices)-see submenu on left, and online at www.travel.state.gov. Basic requirements for U.S. Passport applications include: 1. Proof of U.S. Citizenship (a certified original birth certificate) 2. Proof of Identity (driver’s license) 3. Previously issued U.S. Passport (if you are renewing your passport) 4. 2 recent color photographs. The picture must be face forward and 2” x 2”. Several local businesses provide this service. 5. Appropriate fees must be paid by separate checks: a. processing fee of $25.00 for each passport application payable to: Town of Westwood b. application fee payable to: U.S. Department of State. Amounts vary depending on type of Passport requested. To expedite Passport processing, additional fees will be applied. Every person, adult or child, applying for a U.S. Passport MUST appear in person.
